Wellness Initiative for 2016-17 for the 2018 Benefit Plan year
Wellness Initiative in Brief: The baseline employee portion of the SASD insurance premium is 23% cost share. Employees need to accumulate wellness points to earn a discount on this cost share. If an employee completes all 4 steps of the Health Risk Assessment process and their spouse completes all 4 steps of the process (if they are on SASD insurance), an employee pays only 15.6% insurance premium cost share. If an employee and his/her spouse choose to earn wellness points, the employee can earn greater discounts on the insurance premium cost share. If the employee OR the spouse earns 1250 wellness points, the employee pays 14% insurance premium cost share. If BOTH employee AND spouse earn 1250 points, the employee pays 12% insurance premium cost share.
An employee/spouse can earn a maximum of 1000 points from the Bioscreen(the blood draw). Employees/Spouses must participate in other Wellness Activities listed in the Wellness Program Guide to earn the rest of the points.
2017 insurance premium rates can be found by logging into the SASD homepage, click on depts tab, and the rates are listed under Business & Operational Services.
***ALL POINTS FOR THE 2018 PREMIUM YEAR MUST BE EARNED BETWEEN
NOVEMBER 1, 2016 THROUGH OCTOBER 31, 2017.
