Application Process - Substitute Hearing Interpreters

If you are interested in subbing as Hearing Interpreter for the Deaf and Hard of Hearing, please complete the forms below and mail to:

All forms must be completed before you will be added to the substitute/reserve list.

The forms are:

• Hearing Interpreter Application
• A photocopy of your current Department of Public Instruction (DPI) Educational Interpreter/Deaf or Hard of Hearing License or application.
• Verification of successful completion of Interpreting Program (certificate, transcript, etc.)
• Employment Eligibility Verification (I-9)
  • A legible copy of acceptable identification outlined on the back of the I-9 form
  • Directions for the I-9: Complete Section 1. Attach legible copies of “acceptable
    documents” outlined on the back of the form. You must have either one form of
    identification from List A, OR one form of identification from List B AND one form of
    identification from List C.
• WRS Participation Verification
• W-4 Form
• Employee Access to Networked Information Resources – User Agreement
• Criminal Information Records Check Acknowledgement Form
• Direct Deposit Authorization
