Substitute Application Process - Teachers

If you are interested in Substitute/Reserve Teaching, please submit your resume along with past employment references and complete the attached forms and mail to:

Prior to employment, the Sheboygan Area School District will be contacting past employment references and conducting a criminal background check. Results from both the reference and background checks must be positive.

Please complete and return the following forms:

  • Substitute Teacher Application;
  • Employment Eligibility Verification Form (I-9)
    • A legible copy of acceptable identification outlined on the back of the I-9 form. Directions for the I-9: Complete Section 1. Attach legible copies of “acceptable documents” outlined on the back of the form. You must have either one form of identification from List A, OR one form of identification from List B and one form of identification from List C.
  • WRS Participation Verification form
  • W-4 Form
  • Employee Access to Network Information Resources - User Agreement
  • Direct Deposit Form
  • Criminal Records Check Acknowledgement Form
  • DPI Teaching License
    • A copy of your current teaching license OR
    • A copy of your license application and a copy of the check/money order issued to DPI.
    • If you are degreed, non-certified substitute (minimum of a bachelor's degree, but not in education), you must submit your original application to DPI, original transcripts verifying your degree, fingerprint cards (if applicable), photocopy of your certificate verifying completion of Substitute Training Workshop/Seminar, and a $100 payment to DPI. The Department of Human Resources will complete the application and submit to DPI.
    * Please note that the DPI application process has been changed. Effective December 1, 2013, DPI will no longer accept paper applications. Please see the below link for details regarding the process:

These forms can be downloaded or may be obtained from the Department of Human Resources (Room 3407) on the third floor at 830 Virginia Avenue. All signed forms must be returned to the Department of Human Resources.

The Sheboygan Area School District employs a Secretary for the Substitute Program. Incoming substitute requests are recorded on the District voicemail system. The secretary secures substitute teachers between 5:30 and 7:30 a.m. Substitutes also secure positions via the Internet, at their convenience. If you want to contact the Secretary of the Substitute Program, at any time, please leave a voicemail message at 459-3988 and your call will be returned that evening or as soon as possible.

Application forms are available in the Department of Human Resources or on the DPI website. Completed license applications should be returned to Human Resources for required signatures.
