Prom 2013

Prom is just a few months away- April 6, 2013!  We are planning a very memorable event at the John Michael Kohler Art Center.  Please become familiar with the attached dance contract, complete the following permission form, and bring it with you when you purchase a ticket.

If you are attending Prom and bringing a guest, he/she will also need to complete a permission form & guest pass, return them to the student office, and receive administrative approval.  Please return completed guest pass & permission form to the Student Office no later than April 3rd , 2013.

Tickets will be sold for $20 per person after school in the commons during both lunches and in room 109 before and after school on the dates below.  Be sure to bring your SCHOOL ID AND COMPLETED PERMISSION FORM.  You may not purchase tickets for other North students, only guests.  10th, 11th, and 12th graders are permitted to attend Prom 2013.

On behalf of my son/daughter, I further release any and all claims which he/she may have against the Sheboygan Area School District’s Board of Education, its employees and agents, from any such claims.  I also have read and fully understand the NHS Student/Parent Handbook, Dance Contract, and have discussed these rules and expectations with my son/daughter.

As a parent/guardian I understand that if my child is removed from a dance for being under the influence, it is my responsibility to pickup my child from the dance.  In addition, I agree to the school rules and the rules set forth in this dance contract and by signing below I acknowledge, understand and support the school dance policies.

I agree to remain tobacco, drug, and alcohol free for all school events covered by this contract.  I acknowledge that my failure to comply with these rules will result in my ineligibility for all co-curricular and extracurricular activities including athletics, Senior activities, and possibly Commencement exercises, as well as suspension and possible expulsion.


I understand that these dances are school-related events, and all school rules apply.  I agree to abide by the dance behavior rules set forth in this contract including the school dress code and dance rules for any and all dances including, but not limited to Prom for the 2012 – 2013 academic year. Dance Contract 2013


In order to promote a healthy, safe, and enjoyable evening for all students, the following procedures will be in place for this dance.  All parents and students must read, understand, and sign this agreement.  NHS encourages families to discuss these rules together with their students and to copy this document for future reference.


This is a dance contract for students & guests attending North High School. 

It must be filled out completely. An incomplete form suspected of being forged will NOT be accepted.


1. Parking

ANY vehicle in the parking and attendee drop-off area can be searched at any time. Dance attendees will be held responsible and given consequences for any unauthorized object or for any illegal substances in the vehicle.


2. Coat Check

All coats and loose items much be checked-in at the coat check.  NO CARRY-INS ALLOWED.  Students may be checked for illegal, unauthorized or controlled substances prior to entry to the dance area. Security may conduct “pat downs” and purse searches.  Any student who refuses to comply with searching will be denied entry.


3. Entrance

All students must present a current PHOTO ID at the entrance.  All guests must present a photo ID and complete additional paperwork prior to the dance. Trading or selling of tickets is not allowed. Tickets are non-transferable and non-refundable.


Any student denied entry will not be given refunds and no refunds will be made for removal from the dance.


4. Dancing Expectations (applies on and off the dance floor):

A.  During ALL school-sponsored dances, there will be no behaviors that disrupt the goals of this school-sponsored event.  This includes, but is not limited to, grinding, touching of a sexual nature, and any sexually suggestive gestures.

B.  Attendees will dress in good taste and in a manner that matches the goals of this school-sponsored event. For clarification of this expectation, please consult the Student-Parent Handbook or event advisor (s).

C.  School designated officials will have final authority regarding enforcement of these expectations.


A. Warning from chaperone/administration. Wristband will be removed.

B.  Removal from dance with no refund issued.
       1.  Removal will be by a member of the NHS Administrative TEam or by the event advisor(s) (Student Council/Prom Advisor)

        2.  Students removed from the dance will wait in a separate location until:

               a. Parents are contacted.  If parents cannot be contacted, student will wait until the dance is over. AND

               b.  A ride home from the dance can be arranged (self-drive home is acceptable with parent permission)

   C.  Furture removals from dance stemming from repeat violations of the above expectations will result in a ban from the next dance being held at North High School.

   D.  These guidelines will be STRICKLY enforced by Administration, Dance hosts, and all chaperones.

Please Note: Students who are unsure if their attire meets dress code standards should check the photos on the North High homepage or see Ms. Komaromy prior to the day of the event.  Students not meeting dress code standards will not be admitted into the event; Students have the option to return in proper attire.  Once students are admitted into the dance, if they alter their attire in such a way as to make it violate the specified dress code standards, they will be removed from the dance and parents will be notified.


5. Drugs/Alcohol Suspicions

STUDENTS SUSPECTED OF BEING UNDER THE INFLUENCE OF DRUGS AND/OR ALCOHOL WILL BE SUBJECT TO EVALUATION AND/OR A FIELD SOBRIETY TEST, WHICH CAN INCLUDE A BREATHALYZER TEST.  If a student is determined to be under the influence, a parent/guardian is required to pick up the student from the event.


7. Other policies

  1. No outside food or drink         
  2. No glow sticks, necklaces or any other glow paraphernalia
  3. No lighters or matches
  4. No liquids
  5. No sharp objects


Any forgery of signatures on this contract will result in the student not being able to attend prom as well as other school disciplinary consequences.


Students who violate the terms of this contract are subject to suspension from school and loss of privileges to participate in co-curricular and extracurricular activities, including commencement exercises.


No students will be admitted after 1 hour from the start of the dance unless prior arrangements have been made with Dan Stengel.


Students will not be allowed to re-enter the dance once they leave.
