Attention Student - Athletes: Prevea Sports Medicine is again offering Pre-Season WIAA Sport Physicals. You need a physical, Ms. Timm in the Student Office can assist you. Information on the physicals is located at the Athletic Training Room or in the student Office.

North High Winter Activity Code Information
Attention new students or athletes if you have never participated in an activity while at North you must sign up tp watch a code video with your parent, there are 8 different times in the month of October.
Student who need to be trained in just the concussion part must sign up in the student office. There are 6 times during the month of October. Remember without watching these you will not be cleared for winter sports
If you are currently involved in a sport and plan on participating in a winter sport, you must stop down and pick up a concussion form to be signed by you and your parents and turn that in with your fee in order to be cleared.
Winter sports are just around the corner.
Students who anticipate going out for any sport must have on file at school, before he/she can participate, a completed WIAA physical card signed by a physician. These cards may be picked up in the Student Office at North High School between 7:00 a.m. and 2:30 p.m., or you can click on the links below to print your own copy.
Any student going out for a sport/activity will need the following to get a clearance slip. The clearance slip is mandatory to start practice.
- A completed WIAA physical card or alternate year card
- A completed insurance waiver and activity code agreement signed by the parent and student
- Completed student and parent concussion acknowledgement forms
- $35 activity fee
- A salmon clearance slip will be given to the student once all the items are turned in. Athletes will not be allowed to practice until the salmon slip is turned into the head coach.

WIAA Physical Cards
Students who anticipate participating in any fall sport must have, on file at school before he/she can participate or practice, a completed WIAA physical card signed by a physician. If an athlete had an exam prior to April 1, 2011, another physical exam is required. If an athlete had a physical exam after April 1, 2011, then an "Alternate Year Athletic Permit Card," signed by a parent or legal guardian, must be on file.
