Family Medical Leave

Please use the below 4 steps for leave associated with yourself. Questions? Please call Human Resources at (920) 459-3553 or (920) 459-6460.

1. Employee

  • Notifies principal and Human Resources at least 30 days prior to leave (if the event is foreseeable)
  • Works with principal/supervisor to ensure substitute arrangements
  • Completes SASD’s “Request for Family and Medical Leave of Absence - EMPLOYEE” (available in HR/school office)
  • Returns SASD’s “Request for Family and Medical Leave of Absence - Employee” to Human Resources

2. Physician

  • Completes “Certification of the Health Care Provider for EMPLOYEE’s Serious Health Condition” form and gives to employee or faxes to Human Resources at (920) 459-6461.

3. Human Resources

  • Approves or denies leave based on Wisconsin and Federal FMLA guidelines

4. Employee

  • Notifies building principal and Human Resources of any change in leave status
  • Obtains return to work release from physician allowing them to return to work from leave due to their own serious health condition

  • Works with principal/supervisor to ensure substitute arrangements
  • Completes SASD’s “Request for Family and Medical Leave of Absence - FAMILY” (also available in HR/school office)
  • Returns SASD’s “Request for Family and Medical Leave of Absence - Family” to Human Resources

2. Physician

  • Completes "Certification of the Health Care Provider for FAMILY MEMBER’s Serious Health Condition” form and gives to employee or faxes to Human Resources at (920) 459-6461.

3. Human Resources

  • Approves or denies leave based on Wisconsin and Federal FMLA guidelines

4. Employee

  • Notifies building principal and Human Resources of any change in leave status
