SASD COVID-19 Testing Sites

On-site COVID-19 testing is offered in partnership with the Wisconsin Department of Health Services and COVID Clinic.
Testing is open to students, staff, and families in the Sheboygan Area School District

No Appointment Needed
Pre-Registration is Recommended
No Insurance or ID Needed

To Utilize the Testing Sites:

  • Parents/guardians must be present to test children in elementary and middle school.
  • High school students are required to have parent/guardian consent to be tested. In the event that a parent/guardian cannot be present with a high school student, a consent form must be completed and handed in at the clinic. These forms can be picked up at any SASD high school office or can be printed below.
  • Families of all students will also need to complete a short online registration on COVID Connect, which can be done in advance at https://register.covidconnect.wi.gov/en-US/ or at the door.
    Please note: our SASD testing sites are considered ‘non-public testing sites’.

COVID Clinic is working on getting all of our school sites up and running. Until all sites are open, please check the list of 'Open Testing Sites' to see which locations are open.

SASD COVID-19 Testing Sites

2820 Union Ave

The testing site is located on the west side of the building with a separate entrance near the theater doors.

*The main entrance to Horace Mann is now open and individuals can enter from Union Ave. 

The South High School testing site will temporarily be located inside South High School. Please park in the circle driveway on S. 12th St and enter through Door #8.

- The COVID Clinic testing site will be moving to an outdoor cottage starting Thursday, October 21. The testing site will be located in the small parking lot between the main office and the commons. Please park in the main parking lot on Washington Ave.

South High

Updated October 13 - The North High temporary in-building testing site is only available for North students and staff.

- The COVID Clinic testing site will be moving to an outdoor cottage starting Thursday, October 21. The testing site will be located on the north side of the building (by the practice fields) and will be available to all Sheboygan Area School District students, staff, and families. Follow the access road or sidewalk past Door #1 and proceed around the corner to the testing site. Parking is available near the testing cottage.

Consent Forms for High School Students

High school students are required to have parent/guardian consent to be tested.
