SASD COVID-19 Testing Sites

SASD COVID Clinic testing sites are now open. 

Under new state regulations for the 2022-23 school year, testing is now only open to Sheboygan Area School District students and staff.

On-site COVID-19 testing is offered in partnership with the Wisconsin Department of Health Services and COVID Clinic.

WEEKLY SCHEDULE

Please note that days and hours are limited based on new state guidelines that each site can only be open 12 hours per week.

NOTICE: All COVID-19 Testing Sites will be CLOSED on national holidays.

SASD COVID-19 Testing Site Locations

SASD Central Administration Building

The testing site is located on the south side of the building (west of the main entrance) with a separate entrance near the theater doors.

HMMS

The testing site will be located on the north side of the building (by the practice fields) and will be available to all Sheboygan Area School District students, staff, and families. Follow the access road or sidewalk past Door #1 and proceed around the corner to the testing site. Parking is available near the testing cottage.

South High School's testing site is located in the small parking lot between the main office and the commons. Please park in the main parking lot on Washington Ave.

South High

To Utilize the Testing Sites:

  • Parents/guardians must be present to test children in 4K, elementary and middle school.
  • High school students are required to have parent/guardian consent to be tested. In the event that a parent/guardian cannot be present with a high school student, a consent form must be completed and handed in at the clinic. These forms can be picked up at any SASD high school office or can be printed below.
  • Prior to going to a testing site, families of all students will need to complete a short one-time online registration on COVID Connect 2 at https://covidconnect2.wi.gov/. (This is a new registration system as of August 2022). View Registration Instructions

Consent Forms for High School Students

High school students are required to have parent/guardian consent to be tested. In the event that a parent/guardian cannot be present with a high school student, a consent form must be completed and handed in at the testing site.
