Horace Mann Families – Winter & Spring Sports Are Coming!
Thinking about joining a winter or spring sport this year? Make sure your student’s sports physical is up to date!
Not sure if they need a new physical or just an alternate year form?
- Stop by the office during conferences
- Have your student check in after school or during lunch
- Call us at 920-459-3666 to find out.
If your student is headed in for a physical, be sure to bring the Sports Physical form to be filled out.
