Dear Sheboygan Area School District families,
The 3rd Quarter/2nd Semester of the school year begins on Monday, January 25. If you would like to have your child switch their current enrollment status (hybrid or virtual), please fill out one form for each of your children.
If you do not want to change your child's enrollment status, you do not need to complete this form.
All students who do not complete this form will be assigned to their current enrollment status (hybrid or virtual) for the remainder of the school year.
Those wishing to change their child's enrollment status for the second semester must complete this form by 5 pm on Tuesday, January 12.
We are asking that you commit to hybrid or virtual for the entire second semester as staffing and scheduling shifts need to occur to accommodate enrollment numbers. Any requests to change during the semester will be handled on a case-by-case basis as space allows.
