If your child has a food allergy or condition that affects his/her food intake and he/she plans to eat school meals, please print off the “Special Dietary Request Form” below and have it completed by a licensed medical practitioner (a healthcare provider who can prescribe medication) to request substitute foods/beverages. Please send the completed forms to your school secretary or directly to the School Nutrition Department in person at Central office, or via email to Grace Tesmer (firstname.lastname@example.org).
