Calling all athletes!

Any students interested in participating in one of Urban's interscholastic sports MUST fill out the sign-up form linked below.
Any students new to UMS athletics will need a physical in order to play. If you have a physical on file at UMS from last year, please fill out the required alternative year physical form. Everyone will have to fill out a concussion form and code of conduct form.

These forms are available electronically on Skyward during registration or can be found on the Urban Middle School website.
